Pulmonary Circulation and the Ventilation- Perfusion Ratio

چکیده

The heart drives two separate and distinct circulatory systems in the body: the pulmonary circulation and the systemic circulation. The pulmonary circulation is analogous to the entire systemic circulation. Similar to the systemic circulation, the pulmonary circulation receives all of the cardiac output. Therefore, the pulmonary circulation is not a regional circulation like the renal, hepatic, or coronary circulations. A change in pulmonary vascular resistance has the same implications for the right ventricle as a change in systemic vascular resistance has for the left ventricle. The pulmonary arteries branch in the same tree-like manner as do the airways. Each time an airway branches, the arterial tree branches so that the two parallel each other (Fig. 20.1). More than 40% of lung weight is comprised of blood in the pulmonary blood vessels. The total blood volume of the pulmonary circulation (main pulmonary artery to left atrium) is approximately 500 mL or 10% of the total circulating blood volume (5,000 mL). The pulmonary veins contain more blood (270 mL) than the arteries (150 mL). The blood volume in the pulmonary capillaries is approximately equal to the stroke volume of the right ventricle (about 80 mL) under most physiological conditions.

Changes in the Pulmonary Circulation after Bronchial Occlusion in Anesthetized Dogs

The effects of bronchial occlusion on the pulmonary circulation were studied in anesthetized cats and dogs. Immediately after occlusion, blood flow fell rapidly at normal pulmonary arterial pressure (—71% cats, —54% dogs), and in constant flow perfusion experiments, perfusion pressure rose (+50% cats, +28% dogs). These resistance changes were reversed by vasodilator drugs, alkali, or perfusion ...

Diagnosis of thromboembolic disease: Combined ventilation perfusion lung scan and compression ultrasonography

The clinical management of pulmonary embolism and deep venous thrombosis of the legs are similar and requires prolonged anticoagulation therapy. The standard diagnostic approach in patients suspected of pulmonary embolism is ventilation-perfusion (V/Q) lung scan and compression ultrasonography to detect deep venous thrombosis. This retrospective study analyzed the role of V/Q lung scan an...

Postural changes in pulmonary blood flow in pulmonary hypertension: a noninvasive technique using ventilation-perfusion scans.

To determine whether postural changes in ratio of upper to lower (U:L) zone pulmonary blood flow reflect pulmonary arterial pressures, we used pulmonary perfusion photoscintigraphy to study 12 normal subjects and 10 patients with precapillary pulmonary hypertension (eight classified as "primary" and two as thromboembolic). All patients underwent right-heart catheterization and measurement of pu...

Spurious lung perfusion ventilation defect on planar ventilation-perfusion scan detected by SPECT imaging

Pulmonary embolism (PE) is a preventable cause of morbidity and mortality which needs prompt recognition. Ventilation-perfusion (V/Q) scan is a well-established diagnostic test for evaluation of suspected PE. We report a 32-year-old woman with history of rheumatologic disease and acute dyspnea, who was referred for V/Q scintigraphy. The planar images revealed multiple mismatched defects through...

Regional ventilation in the differential diagnosis of pulmonary embolism.

In the diagnosis of pulmonary embolism by lung scanning, clinical errors of interpretation may arise. Diseases that affect the distribution of pulmonary blood flow, such as pulmonary emphysema and bronchial asthma, may be confused with pulmonary embolism. With the addition of ventilation studies with 133xenon to the perfusion scans, distinct differences appear between patients vith emboli and t...

Gas exchange after pulmonary thromboemoblization in dogs.

Gas exchange following experimental pulmonary thromboembolization was studied with an inert gas elimination technique in 17 dogs. Pulmonary arterial, systemic arterial, and expired gas concentrations of six gases infused intravenously were measured before embolization and 5, 10, 15, 30, 60, and 120 minutes after embolization. Distributions of ventilation-perfusion (VA/Q) ratios were derived fro...
